Performance Improvement Manager

Job Summary: Under the supervision of the Vice President of Performance Improvement, is responsible for the execution of performance improvement activities that align with and support the achievement of agency strategic goals and positive consumer outcomes. Responsible for working with agency staff, contract service providers, service partners, and other stakeholders to ensure quality services and compliance with external requirements. Leads agency staff in the identification, implementation and evaluation of actions directed toward improving individual
and organizational performance, including data collection, analysis, and presentation.
Qualifications: Must have a Bachelor’s degree. Must have a minimum of two years of experience managing performance improvement and risk management functions, preferably in a social service or healthcare setting. Must have certification as a quality or performance improvement associate or manager or demonstrate that he or she is qualified by equivalent education and experience. Must have a proven ability to influence stakeholders together with strong interpersonal skills and a
motivational management style that is successful in influencing change. Must demonstrate project management skills and time management skills. Must have the ability to work effectively within large systems as well as with individuals and small groups. Must have strong oral and written presentation skills. Must have basic statistical knowledge and basic knowledge of MS Excel; MS Access; MS Word; MS Outlook. Must be able to demonstrate an understanding of improvement tools. Familiarity with the following preferred: Ohio Department of Mental Health, Ohio Department of Alcohol and Drug Addiction Services, third party provider standards applicable to reimbursement, Medicare/Medicaid reimbursement, Federal Head Start and Early Head Start, The Ohio Board of Pharmacy, The Joint Commission, The National Association for the Education of Young Children.

Professional III - EAP Clinical Specialist

  • Qualifications: Must have a Masters Degree in social work or a related field and five years direct practice experience, including independent licensure (LPCC, LISW or equivalent). Must have Chemical Dependency experience and obtain SAP qualifications. Must have mental health licensure in the State of Ohio.   • Job Summary: Under the supervision of the Director of Program Operations, and in conjunction with CFC’s Medical Director, this individual is responsible for the organization and management of CFC’s integrated health services project and Substance Abuse and Mental Health Services--Primary and Behavioral Health Care Integration (PBHCI) grant.  Serves as an active team member in CFC’s overall integrated health service planning and implementation.  Works collaboratively with the CFC staff and partner organizations to develop and implement services and programs to improve the quality and efficiency of care supported by the project and sustainability of services beyond the grant period.  The target population for services is individuals with severe and persistent mental illness; many of whom lack basic needs such as food, housing, and other resources.
  • Qualifications: Must have a Master’s degree in Social Science, Nursing, Public Health, Healthcare Administration, or other related field. Project management and supervisory experience-3-5yrs.  Excellent organization skills. Knowledge of the community mental health system required.  Knowledge of primary care practices preferred. Grants management experience strongly preferred, especially federal grants. Program design and implementation experience required. Excellent oral and written communication skills. Computer proficiency required, especially Microsoft Excel. Must be able to plan, organize and prioritize multiple activities and maintain positive relations with a wide variety of people and work collaboratively with all stakeholders.  Flexibility is a critical skill. Custodian (Faith)

  • Job Summary: Daily (or more often if needed) sweeps and mops classroom floors, vacuums all carpeting, scrubs and disinfects bathroom floor, sinks, counter tops, towel holders, stalls, toilets and drinking fountains.  Cleans mirrors and door knobs.  Cleans tables and chairs  in classroom.  Keeps toilet tissue, paper towels, and soap adequately supplied, empties trash and garbage cans and replaces trash liners.  Weekly (or more often if needed): Washes windows and window sills, classroom counters, cubbies and cabinet tops, cleans toothbrush rack, and telephone(s), assists if needed with set up and take down of classroom furniture.  Cleans fingerprints from walls and woodwork. Periodically as needed: Keeps halls and other areas (i.e. gym) used by Head Start staff clean, strips and waxes floors, notifies maintenance of maintenance of major repair needs, assists with snow removal and salting walks, performs minor repairs such as removal of flaking paint and repainting, participated in fire/tornado drills. Performs other related duties as assigned. 
  • Qualifications: Prior related experience.  High school diploma or GED.  Must be able to meet Head Start and ODJFS licensing requirements, and the physical demands of the position.
